Where can I get a copy of my birth certificate?
Copies of birth certificates are available from the Registrar of Vital Statistics in the village, town or city (or Monroe County) where the child was born or from the Vital Records Section of the New York State Department of Health. The Village Clerk is the Registrar of Vital Statistics for the Village of Avon.It does not matter where the parents lived at the time of the birth. Births are registered in the village, town or city (or Monroe County) where the birth took place.
Who is eligible to obtain a copy of a birth certificate?
- The person named on the birth certificate
- A parent of the person named on the birth certificate (requesting parent's name must be on the birth certificate)
- Only by order of a New York State Court may a spouse, child or other person obtain a copy of birth certificate
How much does it cost?
The fee for obtaining a copy of a birth certificate from the Village is $10 or $30 from the New York State Department of Health.
How do I request my birth certificate?
- Download the Application to Local Registrar for Copy of Birth Record form or pick-up an application at the Village Clerk's office.
- Turn-in the completed application, the $10 fee and show identification at the Village Clerk's office.
- Turn-in your application, a photocopy of identification and payment, if requesting a birth certificate by mail.

Where can I get a copy of a death certificate?
Copies of death certificates are available from the Registrar of Vital Statistics in the village, town or city where the death occurred or from the Vital Records Section of the New York State Department of Health. The Village Clerk is the Registrar of Vital Statistics for the Village of Avon.Deaths are registered in the village, town or city where the death occurred. It does not matter where the deceased lived at the time of death.

- Sons, daughters, parents, the CURRENT spouse of the deceased or their lawful representative
- An applicant with a legally documented right or claim
- An applicant with a medically documented need
- An applicant with a NYS Court Order attesting to a necessity
- An attorney representing someone eligible to obtain a copy of a death certificate
- A government agency for official purposes

What records are available?
- Birth, death & marriage records begin in 1881.
- Genealogical records are not certified.
- Birth records must have been on file for 75 years or more and the person named on the record must be known to the applicant to be deceased.
- Death records must have been on file for 50 years or more.
- Marriage records must have been on file for 50 years or more and both parties named on the record must be known to the applicant to be deceased.
What is the cost?
The fee schedule depends on the years being searched: 1-3 years is $22; 4-10 years is $42; 11-20 years is $62; 21-30 years is $82; 31-40 years is $102; 41-50 years is $122; 51-60 years is $142; and 61-70 years is $162.
How do I request a record?
Download the
General Information and Application for Genealogical Services or fill-out the form at the Village Clerk's office. Turn-in the completed form, the appropriate fee and show or send-in a photocopy of acceptable identification.
